PERKAYA was elected by the Honorable Royal of Terengganu, Dato 'Seri Amar Tan Sri Wan Mokhtar bin Ahmad Terengganu Chief Minister during his government since 1970 until 1999 at the State Government Exco meeting on April 8, 1979. He requested YBM Dato' Tengku Mahmud bin Mansor, Chairman of the General Welfare and Conservation Committee Terengganu State at that time to deal with the suffering and hardship of some childrens who were called 'orphans' due to family poverty, neglected and inadequate basic services and facilities. Hence, from the results of the during meetings and several discussions were conducted involving various strategic groups to find ways and approaches that best address towards this issue. Establishing an orphanage welfare organization is the best method available.

The first letter of the establishment of PERKAYA was organized and administered by YBM Dato 'Haji Tengku Mahmud bin Mansor, Exco Member of the State Government.
November 29, 1980
A General Meeting of the Establishment of PERKAYA was held and a Protem Committee was formed chaired by YBM Dato 'Haji Tengku Mahmud bin Mansor.
June 24, 1980
A provisional permit has been issued by the Registrar of Societies of Malaysia for operation.
December 4, 1980
ROC is registered under the Establishment Act 1966 with registration number 418 (Terengganu) under its official name, Terengganu Orphan Welfare Organization.
